Description:
Homestead Family Medicine is expanding and we are seeking a compassionate, motivated, and forward-thinking physician to join our growing direct primary care practice. As part of our team, you'll experience the freedom and satisfaction of providing care without the constraints of insurance-based systems. You'll manage a smaller patient panel with extended clinic visits allowing time to think deeply, treat thoughtfully, and provide truly personalized care.
Responsibilities:
Diagnose, treat, and manage acute and chronic illnesses for patients of all ages
Perform detailed histories and physical examinations
Order, interpret, and act on diagnostic tests (labs, imaging, etc.)
Develop and manage treatment plans for chronic conditions such as diabetes, hypertension, asthma, and depression
Monitor patient progress and adjust care plans accordingly
Provide care for minor injuries, infections, and other urgent medical concerns
Determine the need for specialty referral or higher level of care when necessary
Collaborate with specialists, behavioral health providers, and allied health professionals
Coordinate transitions of care (e.g., hospital discharges, specialty follow-up)
Educate patients and families on disease prevention, health maintenance, lifestyle modification, and treatment adherence
Maintain accurate, timely, and thorough documentation in the electronic health record (EHR)
Perform common office-based procedures (e.g., joint injections, skin biopsies, IUD placement, suturing, cryotherapy)
Provide care for patients struggling with addiction to drugs and/or alcohol
Provide home care (“housecall”) as needed
Maintain competency in skills appropriate to the practice scope

Requirements
Qualifications:
Board-certified or board-eligible in Family Medicine
Oregon medical license (or ability to obtain)
Passion for patient-focused, high-integrity care
Excellent interpersonal and communication skills
Alignment with the values of direct primary care
